Eldercare

Horizontal Rule

Nearly one out of three Americans is providing care for a family member. If you are responsible for caring for an elderly relative, there is support available to you through our Employee Assistance Program (EAP).

Eldercare Referrals

The EAP eldercare specialists gather information about your eldercare situation in order to determine which services are needed. An updated database of eldercare resources is maintained for use in arranging referrals. The EAP care specialist will save you time and give you peace of mind by assessing the appropriateness of facilities and ensuring that there are available openings before providing you with any referrals.

Referrals are available for the following services:

  • Care Options and Living Arrangements
  • Caregiver Issues and Concerns
  • Professional Geriatric Care Management
  • Meal Delivery and In-home Services
  • Transportation Services
  • Hospice Services
  • Medicare, Medicaid and Social Security
  • Respite Care
  • Funeral Planning
