Survivor Benefits & Support
Some of the kinds of benefits payable to surviving beneficiaries upon the death of an employee include:
- Life Insurance (FEGLI)
- Unpaid Compensation: the employee's last pay check, including annual leave balance SF-1152
- Federal Employees Retirement System (FERS), SF-3102
- Civil Service Retirement System (CSRS), SF-2808
- Thrift Savings Plan (TSP), TSP-3
It is necessary to designate a beneficiary if:
You want benefits to go to a person, firm, organization, or other legal entity not listed in the order of precedence; you want benefits to be paid differently than the order of precedence; you want benefits to go to a trust, for example, one you have established for your minor children; or evidence of a valid marriage or dissolution of a marriage is not readily available. Important Things to Remember about Designations:
- Only the insured can sign the designation of beneficiary. Exception: If you assigned your FEGLI insurance (using an RI 76-10 Assignment form), only the assignee(s) has(have) the right to make a designation.
- The Employee Benefits Branch, the TSP, and SAMBA (for BF USSS0220) must receive the completed form before you die. A designation delivered on a weekend or Federal holiday is not "received" and is not valid until the next workday. If you die before your appropriate office receives the new designation, the entity will pay benefits according to the next prior designation on file or under the order of precedence, if there is no prior designation.
- A witness cannot be someone you are naming on the form as a beneficiary.
- Be sure to keep your designation up to date. If you marry or divorce, complete a new form. If your beneficiary's address changes, complete a new form.
The following is a list of all Designation of Beneficiary Forms. These forms can be updated at any time and payment will be processed according to the most recent form on file. You should always receive confirmation that your designation(s) was received and keep a copy of the latest certified/received form(s). Without a completed form, payment will be issued in order of precedence.
The SF-1152, SF-2823, SF3102, and SF-2808 should be sent to the Employee Benefits Branch via email to BPR-Benefits@usss.dhs.gov. Two persons must witness your signature on these forms. These witnesses must sign the form and give their addresses.
- SF 1152- Designation of Beneficiary Form, Unpaid Compensation of Deceased Civilian Employee, SF1152. This form will provide payment of any unused annual leave and final salary check to listed beneficiary.
- SF 2823- Designation of Beneficiary, Federal Employees Group Life Insurance Program (FEGLI) SF2823. You cannot designate beneficiaries if you have assigned your insurance.
- SF 2808 - Designation of Beneficiary, Civil Service Retirement System (CSRS) SF2808.
- SF 3102 -Designation of Beneficiary, Federal Employees Retirement System (FERS) SF3102.
- TSP - Designation of Beneficiary must be completed by accessing your TSP account through the TSP website.
- Secret Service Employee Benevolent Fund, BFUSSS0220 . This form should be faxed directly to SAMBA. The fax number is on the form.
The table (below) provides an overview of the benefits that may be payable or available when a federal employee dies. The table should be read in conjunction with applicable laws and regulations (United States Code (USC) and Code of Federal Regulations (CFR)). Eligibility and payment determination are made by the agency responsible for adjudicating the claim for benefits. Several benefit programs require a determination that the employee's death resulted from an injury or illness sustained while in the performance of the employee's official duties (noted in “Line-of-Duty Requirement” column) and may limit eligibility further to deaths resulting from specific causes/circumstances.

Federal Employees Retirement System (FERS) Basic Employee Death Benefit (BEDB) (95% of the DHS workforce is covered by FERS. FEHB - Temporary Extension of Coverage, Conversion and Temporary Continuation of Coverage (TCC) | Continuation of Health Insurance | • When the employee has a Self and Family FEHB enrollment or a Self Plus One FEHB enrollment, but the family members are not eligible to continue FEHB coverage as survivor annuitants (see above), the family members will be eligible to: o Continue FEHB coverage free for 31-days o Convert to a non-FEHB plan with the current carrier, o Elect TCC coverage (paying 102% of the premiums) for up to 36 months. • These options would apply to: o Surviving spouses who are not eligible for the Basic Employee Death Benefit or an annuity (generally those spouses who were married to the employee for less than 9 months and no child was born of the marriage) o Children who have reached age 18 (if not in school or disabled) and children who have reached age 22.
